Sports Physical Forms

If your child is going to need a sports physical form completed during your appointment with us, please notify the front desk when you check-in. If your child has had a well child appointment in the past 12 months, you do not need to come in for another exam to receive a sports physical form, with the exception of children who have tested positive for COVID-19. Any COVID positive patients will have to have a cardiac exam in the office in order for us to complete their sports physical form.

All Other Forms

If you have a form you need completed for your child, please bring it to the office with any pertinent instructions. Please allow 5-7 days for form completion and all forms are subject to a $25 fee.

Appointment Scheduling

Our goal is always to meet your expectations as far as your ability to get an appointment for your child when it is convenient for you; however, that is not always possible. We have extended hours during the week and Saturday hours in our New Albany office, but even then, we may not be able to give you exactly what you want for an appointment date and time.

Evening hours through the summer are available for well child/check-up appointments, but those fill up quickly – often a month or more in advance. After school starts, our evening hours are dedicated largely to sick appointments as children share more germs in school and group settings. If you know your child will need a check-up after 4 pm, we suggest you call well in advance to schedule that appointment for the summer.
